Abnormal Uterine Bleeding

When menstrual bleeding occurs that is abnormal in duration, amount or frequency, it may be labeled abnormal uterine bleeding, or AUB. This is a common occurrence among women of all ages and can be caused by a wide variety of factors. Normal menstrual bleeding occurs for 5-7 days monthly as the uterine lining sheds until menopause. Normal uterine bleeding or periods should occur no less than every 21 days and not more than 35 days apart – most normal menstrual cycles are between 21-35 days long. Bleeding that occurs outside of the normal menstrual cycle or in heavier amounts can be considered abnormal.

Abnormal uterine bleeding includes heavy periods, spotting between periods, spotting after intercourse and any uterine bleeding after menopause. If AUB occurs, there are many possible causes. These can include:

  • Hormonal imbalance
  • Fibroids or polyps
  • Anovulation (lack of ovulation)
  • Medications
  • Bleeding disorders
  • Adenomyosis, PCOS
  • Miscarriage or ectopic pregnancy
  • Cancer

The most common cause of AUB is hormonal imbalance. Many women with heavy periods or bleeding between periods respond to hormone treatments, including birth control to regulate their menstrual cycle and manage their abnormal bleeding. If hormonal regulation does not achieve a normal cycle, other treatments may be recommended.

Treatment for AUB

The first step in prescribing treatment for AUB is determining the cause. If hormonal imbalance is not the cause, our GYN providers will perform testing and exams to find the reason for the abnormal bleeding. The cause of AUB can be elusive, requiring diagnosis by exclusion of possible causes. There are many possible treatments for structural abnormalities and conditions that can cause abnormal uterine bleeding. Whenever possible, non-invasive treatments are recommended, but some patients may require surgical procedures when other treatments are ineffective.
